A woman with multiple personalities has almost thrown up in the witness box, while reliving alleged child sexual abuse by her father.

The accused, who can only be referred to as SN, is standing trial in Newcastle District Court, after pleading not guilty to abusing two of his children, including LN.

LN has been diagnosed with dissociative identity disorder and is being cross examined as three alternate identities, also known as alters.

One of her alternate personalities, a defiant 13-year-old girl, was sworn in on Wednesday.

Defence barrister Garry Sundstrom questioned the alter about the evidence she gave about the alleged use of sex toys.

"I put to you that didn't happen, you weren't ever asked to choose a sex toy were you?" he said.

The girl snapped back and stared.

"You're wrong, I put it in the Cabbage Patch bassinet, you know what I said," she said.

'I am about to vomit'

The girl, LN, then talked about being forced to go to the toilet after alleged sexual abuse "to stop infection that would make people ask questions". 

The defence barrister also dismissed allegations LN's father had tried to choke her alter, or that SN had suggested the world would end because of his daughter.

"I can still feel his hands around my throat … you're wrong," she said.

Shortly afterwards, she looked woozy and said "I am about to vomit".

Her witness support person left her side to get a sick bag.

"I am just trying not to vomit, Dad said the world is going to end," LN's alter said.

"I put to you neither parent said that to you," Mr Sundstrom said.

"You're wrong," she said.

Judge Ian Bourke told jurors a break was needed and before they left the courtroom, LN, as her alter, said "I am going to be sick".

Earlier on Wednesday, LN, as the altered personality, spoke about being allegedly forced to touch her father in the bath, an allegation the defence also vehemently rejected.

Mr Sundstrom also put to LN, as the girl alter, that her father never threatened to kill her.

"You lie, they tried with guns and knives to kill me,  you lie, so you're a liar," she said.

"I suggest they didn't say that to you," Mr Sundstrom repeated.

"You say I am wrong … you're wrong," she said.

Older alter in witness box

On Tuesday, LN entered the witness box as an older alter, a self-assured, forthright 38-year-old woman, whose role was that of carer and protector of LN's baby sister.

"Our brain saw a need and I was created … the abuse is why I was created," the alter said.

The older woman alter said she knows she is required when "protection is needed", which was prompted by "triggers and certain actions of people".

"Touching a leg, unwrapping of a condom, raised voices, glass breaking, children screaming … threats draw my attention," she said.

During defence questioning, the woman berated a member of the defence's legal team, claiming the solicitor laughed.

"Your subordinate laughed, sexual assault is not funny … nothing in this situation is funny," LN's woman alter said.

"I do not appreciate being looked down upon."

LN's sibling, NT is yet to give evidence.
